Job description

Responsibilities:
  • The Faith Community Nursing (FCN) Coordinator supports the development, implementation, and evaluation of faith community nursing programs in assigned communities.
  • Serving as a liaison between the healthcare system, local faith communities, and the community at large.
  • The coordinator guides both paid and volunteer FCNs in providing holistic care that integrates physical, emotional, and spiritual well-being.
  • Key responsibilities include recruiting and orienting FCNs, assessing health needs, coordinating and performing wellness screenings and education events, as well as presentations to nurses and clergy.
  • The coordinator also maintains program data, ensures compliance of data collection from FCN Volunteer and self reporting on Community Benefit Form.
  • Fosters partnerships with congregations and community organizations to align services with BayCare's health priorities and Community Health Needs Assessments (CHNAs).
  • The faith community nurse coordinator is responsible to coordinate care of the clients enrolled in the FCN Transitional Care Program.
  • Responsible for transportation between facilities and to community events.
  • Other duties as assigned.

Minimum Qualifications:
Required Education: Associate's degree in Nursing or Diploma Nursing. Preferred Education: BSN (Bachelor of Science in Nursing).
Required Certification: BLS (Basic Life Support) and RN (Registered Nurse).
Required Experience: Minimum 3 years of direct patient care experience. Working knowledge and computer proficiency (Microsoft suite).
