We're looking for a compassionate, experienced LPN or RN to serve as our Health & Wellness Director—someone who leads with integrity, builds strong relationships, and is passionate about providing exceptional care.

What You'll Do
  • Lead, mentor, and support the wellness team while promoting a culture of excellence.
  • Oversee resident assessments, admissions, service plans, and ongoing care coordination.
  • Ensure residents receive exceptional, person-centered care that promotes independence and quality of life.
  • Communicate effectively with residents, families, physicians, pharmacies, and healthcare providers.
  • Supervise medication management and ensure safe medication administration.
  • Maintain compliance with state regulations, licensing requirements, and company policies.
  • Monitor documentation, resident records, and care plans for accuracy and compliance.
  • Respond to resident concerns, emergencies, and changes in condition.
  • Identify potential risks and implement proactive care and safety plans.
  • Participate in Manager on Duty rotations and provide leadership support as needed.
What We're Looking For
  • Active Nebraska LPN or RN license required.
  • Minimum of 1 year of experience working with seniors or in long-term care, assisted living, or a related healthcare setting.
  • 2+ years of leadership or management experience preferred.
  • Strong knowledge of medications, treatments, and senior health needs.
  • Excellent communication, organizational, and problem-solving skills.
  • Comfortable using electronic documentation and computer systems.
