Job description

Shift Hours: 7a-7p 

Pay Range: $25-29/Hour

• Rate of pay is based on experience
• Must have current LPN license
• Prepares, monitors, and administers medication to Residents at scheduled intervals
• Assist Residents diagnosed with diabetes at regular intervals
• Review all notes in the Resident Service Plans Binder, Resident Communication Logs, discussing all pertinent issues with Resident Assistant staff, Resident Services Director, and Executive Director
• Review Medication Administration Records for accuracy
• Review changes in the Resident Service Plans and CareSteps assessments
• Complete Comprehensive Assessments, Physical Self-Maintenance Screen, and Behavioral Screens for all Residents
• Responsible for notifying the Pharmacy upon Resident move-out (including death) or admittance to the Hospital so that Pharmacy can stop medications
• Create a new plan of care for any Resident returning to the Community from the Hospital
• Perform regular audits on all PRN and bulk item medications, ordering from pharmacy when needed
• Perform regular audits of the controlled medications and investigate any discrepancy
• File Residents files and Medication Checklist
• Update Resident Service Plans Binder
• Make medical appointments and schedule transportation for Residents, coordinating schedules with the Resident Services Director/Resident Services Coordinator to ensure there are no conflicts
• Maintains daily communication (both written and verbal) with Executive Director
• Familiar with gerontology and an overall view of health care delivery, including State regulations.
• Must have an overall “caring” attitude
• Excellent verbal and written communication, including listening skills
• Must have ability to supervise others
• Must be able to lift 75 pounds unassisted and 125 pounds per person in a two-person lift
• Will be regular in attendance, with the right attitude and attire
• Valid driver’s license in the state of employment required
