Evening Shift Licensed Practical Nurse (LPN)

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team at Caledonia Senior Living & Memory Care in North Riverside, Illinois. As an LPN, you will play a vital role in providing high-quality care to our residents, while contributing to a collaborative and supportive work environment. Responsibilities:

  • Provide direct care to residents, including administering medications, wound care, and personal care

  • Develop and implement individualized care plans in collaboration with the interdisciplinary care team

  • Monitor and report changes in residents' condition to the nursing team and other appropriate staff members

  • Collaborate with other departments, such as social services and activities, to ensure comprehensive care for residents

  • Participate in quality improvement initiatives and maintain accurate and up-to-date records

  • Demonstrate a commitment to excellence in patient care and a passion for working with seniors

Requirements:

  • Current LPN license in the state of Illinois

  • Basic Life Support (BLS) certification

  • Ability to work effectively as part of a team and communicate effectively with residents, families, and other staff members

  • Strong observational and assessment skills, with the ability to identify changes in residents' condition

  • Ability to lift, transfer, and move residents with assistance, as needed

  • Basic computer skills and familiarity with electronic medical records
