Bliss Reformation and Wellness Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a remote capacity. The position allows for a remote working arrangement, granting you the flexibility to manage your work from home while still being an integral part of our healthcare team. Our hours are primarily from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours as necessary, based on client needs and your availability. • Conduct comprehensive assessments of clients, particularly seniors, to identify their psychosocial needs based on emotional, mental, and social factors.
• Collaborate with clients and their families to create individualized care plans that reflect their unique goals and preferences, ensuring their concerns are addressed effectively.
• Provide counseling and emotional support, guiding clients through life transitions and challenges related to aging, health issues, or family dynamics.
• Advocate for each client's rights and access to community resources, facilitating connections that ensure clients receive the best possible support.
• Engage with interdisciplinary teams—including nurses, medical providers, and caregivers—to coordinate care delivery and share important client information.
• Regularly monitor clients’ well-being through virtual visits or scheduled follow-ups, assessing living conditions and addressing any emerging safety issues.
• Facilitate effective communication and conflict resolution among family members involved in care, promoting collaborative support for the client.
• Educate clients and their families about community resources, support groups, and educational programs to enhance social engagement and improve quality of life.
• Document all client assessments, care plans, interactions, and progress notes accurately in compliance with regulations and ethical standards.
• Stay current with best practices in social work, continuing education requirements, and any operational changes within community health settings.

The primary focus of this role is to support senior clients, providing critical insights into their mental health needs and advocating for their overall wellness. We recognize and address the unique struggles seniors and their families face, including issues related to aging, mental health, and social isolation. Your work will directly contribute to enhancing their emotional resilience and quality of life, promoting a sense of agency and self-management.

• Master's degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.
• A minimum of two years of relevant experience in a social work capacity, preferably within geriatric or community health settings.
• Strong clinical knowledge of social work principles, particularly as they apply to aging populations and available community resources.
• Exceptional skills in assessment, counseling, and problem-solving, with a high degree of empathy for clients.
• Proficient communication and interpersonal skills, fostering effective collaboration with a variety of professionals.
• Ability to navigate electronic health record (EHR) systems and other digital platforms effectively.

While not mandatory, experience within a community health clinic setting or direct exposure to gerontology is advantageous. Familiarity with local resources and support programs for seniors will further enhance your ability to advocate effectively for clients.

This role offers a competitive salary range of USD 90,000 to 100,000 per year, commensurate with experience.
