Guided Behavioral Reflection Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a remote capacity. This full-time position is integral in providing specialized mental health support to our clients, primarily focusing on seniors and their families. The successful candidate will assess and address the psychosocial needs of these individuals while advocating for their well-being.

This role is a full-time remote position, allowing for flexibility in managing your schedule while serving our clients. Our operational hours are Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend shifts based on client needs. The ability to remain on-call for urgent situations is key to providing continuous support. As a member of our community health clinic, you will work in an atmosphere that values collaboration, innovation, and respect.

• Conduct comprehensive assessments that consider the emotional, mental, and social factors affecting seniors, ensuring a holistic understanding of their needs.
• Develop personalized care plans by collaborating closely with clients and their families, focusing on their unique goals and preferences.
• Provide counseling and emotional support to clients, assisting them through significant life transitions and challenges.
• Support and advocate for clients’ rights, ensuring they have appropriate access to community resources and services.
• Collaborate with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to coordinate client care effectively.
• Monitor client well-being through regular visits, assessing living conditions, and addressing potential safety concerns.
• Facilitate family communication, helping to resolve conflicts and promoting a supportive environment for the client.
• Educate clients and families on available resources, support groups, and programs that can enhance their quality of life.
• Maintain thorough documentation of assessments, care plans, and progress notes in compliance with legal and ethical standards.
• Stay updated on best practices in social work, ensuring compliance with relevant policies and guidelines.

• A Master’s degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).

• In-depth knowledge of social work principles, aging issues, and community resources tailored for senior clients.
• Excellent communication skills and a compassionate approach toward client care.

• Previous experience working within multidisciplinary teams.
• Demonstrated ability in crisis intervention and conflict resolution.
• Familiarity with electronic health record systems and documentation processes.

This position offers a competitive salary range of USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
