• Conduct comprehensive psychosocial assessments through detailed interviews, patient observations, and thorough reviews of medical records to identify emotional, social, and environmental factors that may impact recovery.
• Provide compassionate counseling and support to individuals and families, addressing adjustment issues connected to illness and promoting emotional well-being during challenging times.
• Collaborate closely with the rehabilitation team to develop and implement individualized treatment plans that are sensitive to the psychosocial needs of each patient.
• Assist patients in navigating community resources, including connecting them with support groups, vocational rehabilitation services, and housing assistance programs.
• Educate patients and their families about available community resources and post-rehabilitation services fostering successful long-term recovery.
• Engage in thorough discharge planning activities ensuring a smooth transition for patients returning home or to other care environments, considering their psychosocial needs and ongoing support requirements.

Our clinic serves a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. By focusing on both emotional and practical aspects of recovery, you will play a pivotal role in facilitating patients' successful reintegration into their communities and enhancing their overall quality of life.

• A Master’s degree in Social Work from an accredited institution, demonstrating a robust understanding of social work principles and practices.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory to practice in this role.
