Key Responsibilities
  • Conduct comprehensive psychosocial assessments through detailed interviews, patient observations, and thorough reviews of medical records to identify emotional, social, and environmental factors that may impact recovery.
  • Provide compassionate counseling and support to individuals and families, addressing adjustment issues connected to illness and promoting emotional well-being during challenging times.
  • Collaborate closely with the rehabilitation team to develop and implement individualized treatment plans that are sensitive to the psychosocial needs of each patient.
  • Assist patients in navigating community resources, including connecting them with support groups, vocational rehabilitation services, and housing assistance programs.
  • Educate patients and their families about available community resources and post-rehabilitation services fostering successful long-term recovery.
  • Engage in thorough discharge planning activities ensuring a smooth transition for patients returning home or to other care environments, considering their psychosocial needs and ongoing support requirements.
Patient Population / Client Focus

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution, demonstrating a robust understanding of social work principles and practices.

Compensation and Benefits
