As a Clinical Social Worker at Summit Pacific, you will:

• Conduct detailed psychosocial assessments to determine the emotional, social, and financial needs of patients, tailoring interventions accordingly.
• Develop and implement individualized care plans that address not only the healthcare needs but also the life challenges faced by patients.
• Provide both individual and group counseling sessions to support patients dealing with issues such as grief, adjustment to illness, and coping strategies.
• Collaborate with medical teams to ensure an integrated approach to patient care, improving overall outcomes and patient satisfaction.
• Coordinate with community organizations to facilitate post-discharge services, ensuring a seamless transition for patients back into their daily lives.
• Deliver educational resources to patients and families regarding treatment options and available support services.
• Engage in crisis intervention when necessary, providing immediate emotional support and resource connection.
• Maintain accurate and thorough documentation of patient interactions in compliance with both hospital policies and legal standards.
• Participate in hospital committees, providing social work expertise to influence policy and practice improvements.
