The Clinical Social Worker (LCSW) position at Western Mind Care Hub represents a critical role in our commitment to enhancing the mental health and emotional well-being of our clients in the hospice care setting. In this remote role, you will be part of a supportive and collaborative team that values communication and teamwork. Our hospice center provides a compassionate environment where individuals receive specialized care. Your contributions will be supported by a range of tools including virtual therapy platforms and electronic health record systems to facilitate effective telehealth services. You will have the flexibility of a full-time schedule while being accessible to clients, including some evening and weekend hours, to accommodate their needs.

• Conduct thorough psychosocial assessments to understand clients’ mental health concerns, coping mechanisms, and family dynamics, ensuring comprehensive emotional support.
• Provide individual, family, and group therapy sessions utilizing various evidence-based modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care approaches.
• Develop, implement, and regularly review individualized treatment plans based on client assessments, goals, and progress, adjusting interventions as necessary.
• Work collaboratively with interdisciplinary teams, including psychiatrists and medical professionals, to enhance the quality of care and ensure coordinated treatment efforts.
• Offer immediate crisis intervention and support during critical moments, assessing risk and working with clients to develop safety plans.
• Document all client interactions, assessments, and treatment plans in real-time using electronic health records while adhering to ethical and legal standards.
• Facilitate engaging and informative group therapy sessions, psychoeducation classes, and support groups, encouraging client interaction and mutual support.

Your primary focus will be on clients facing end-of-life issues and their families, providing a compassionate understanding of their emotional, psychological, and social needs. You will work with a diverse client base, attending to various mental health conditions such as grief, anxiety, and depression, using your expertise to foster a supportive therapeutic environment.

• A Master’s degree in Social Work from an accredited institution, providing the foundational knowledge required for effective practice.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) to provide therapeutic services within Texas.
• A minimum of three years of experience in a mental health or outpatient setting, with a strong focus on therapeutic techniques suited for clients facing significant life challenges.
• Proficient in evidence-based therapeutic modalities and possess a thorough understanding of mental health treatment options, interventions, and best practices.
• Excellent interpersonal and communication skills that enhance your ability to build rapport with clients and collaborate with colleagues in a remote setting.

• Experience working with hospice or palliative care clients, particularly in support of mental health needs during the end-of-life process.
• Familiarity with telehealth technologies and practices to effectively reach clients in remote or home-based settings.
• Experience conducting group therapy and community outreach programs targeting mental health awareness and support.
