Evening Registered Nurse (RN) - Home Health$10,000 Sign-On Bonus

The Christ Home Health Care

Territory: Cincinnati, Ohio-Downtown, Queens City, Mason, Mainville, W.Chester, Terrace park, Blanchester, Morrow, East End, Mt. Washington


Schedule: 3 days per week
Hours: 2:00 PM - 9:00 PM

As an RN, you will have the opportunity to:

  • Provide one-on-one patient care
  • Work at the top of your nursing license

  • Provide home-based nursing care as directed by the attending physician
  • Work as part of an interdisciplinary team focused on compassionate, quality care and positive patient outcomes
  • Interact with patients and their families throughout the recovery journey
  • Help patients restore their health in the comfort of their own homes

How You'll Work

You'll provide skilled nursing care in patients' homes as directed by the attending physician.

You will work autonomously, so you should be comfortable managing your time efficiently and working independently.

You'll be responsible for all practices and duties within the scope of practice outlined by the state.

Major Areas of ResponsibilityPlan of Care
  • Observe and monitor patient conditions
  • Perform OASIS assessments
  • Develop individualized care plans
  • Adjust care plans as patient needs change

Minimize Patient Risk
  • Administer medications as prescribed by the physician
  • Help decrease re-hospitalizations
  • Prioritize visits for high-risk patients
Continuity of Care
  • Manage multidisciplinary care as applicable
  • Promote continuity of care
  • Coordinate appropriate admissions, transfers, and discharges
Family Educator & Advocate
  • Educate patients and families about disease processes
  • Use teach-back methods to ensure patient and family understanding
  • Support patients and families throughout their care journey
Policies & Documentation
  • Complete clinical documentation according to agency protocols
  • Follow Medicare and federal guidelines
  • Collaborate with the interdisciplinary team and physicians to ensure optimal patient care
Rules & Regulations
  • Understand and follow agency policies, procedures, rules, and regulations
  • Communicate changes in schedule or availability to schedulers or supervisors
Operations
  • Attend in-service training
  • Attend mandatory agency meetings

  • Able to follow directions and established procedures

Requirements
  • Current Registered Nurse (RN) license in
