• Conducting comprehensive assessments of clients’ emotional and psychological needs to inform treatment planning.
• Implementing evidenced-based behavioral management strategies tailored to individual client circumstances.
• Providing individual, family, and group therapy sessions that facilitate healing and resilience among patients and their families.
• Collaborating with a team of healthcare professionals to develop and review patient care plans and assess outcomes effectively.
• Equipping families with practical resources and strategies that promote positive behavioral changes and support long-term wellness.
• Maintaining meticulous clinical documentation in accordance with state regulations and best practices.
• Staying updated on the latest social work methodologies and incorporating new approaches that enhance patient care.

Your primary focus will be on clients within hospice care, many of whom experience complex emotional challenges related to end-of-life issues. Understanding their needs requires sensitivity, respect, and a capacity for providing culturally competent care. By fostering supportive relationships, you will help clients navigate grief, loss, and transition, allowing them to find meaning in their experiences and improve their quality of life.

• A valid and active Master's degree in Social Work (MSW) from an accredited institution.
• Current Licensed Clinical Social Worker (LCSW) credential issued by the state of California.
• A minimum of two years of clinical experience, preferably in mental health or hospice settings.
• Proficiency in Microsoft Office Suite and experience with electronic health record (EHR) management systems.
• Strong communication skills and the ability to build rapport with diverse populations.

• Experience working in hospice or palliative care environments is strongly preferred.
• Demonstrated success in using evidence-based therapeutic modalities such as CBT, DBT, or solution-focused therapy.
• Prior involvement with interdisciplinary teams that dynamically impact patient care outcomes.
