The Health Information (HI) Data Integrity Specialist is responsible for maintaining the integrity of the Hackensack Meridian Health (HMH) Master Patient Index and facilitates synchronization with other HMH applications as needed. The main functions, in accordance with hospital policy and/or regulatory standards, are reviewing patient health information, performing patient merges, and completing chart corrections and contact moves.

This position is fully remote. Experience with Epic and HIM/Medical Records is preferred. 


A day in the life of a Health Information (HI) Data Integrity Specialist  at Hackensack Meridian Health includes:

  • Monitors work queues of potential duplicate patients as well as Information Technology (IT) tickets assigned to the team to ensure those that need to be updated and corrected.
  • Investigates patient information in multiple systems and compares patient identification and signature to determine if a merge or unmerge is appropriate.
  • Achieves department productivity standard of 3.0 merges per hour.
  • Investigates, analyzes, verifies patient information utilizing multiple hospital computer applications to perform Chart Corrections to maintain the accuracy and integrity of the patient record.
  • Investigates, analyzes, verifies patient information utilizing multiple hospital computer applications to perform Contact Moves from one patient to another to maintain the accuracy and integrity of the patient record.
  • Investigates, analyzes, verifies patient information utilizing multiple hospital computer applications to perform Document Corrections to maintain the accuracy and integrity of the patient record.
  • Investigates, analyzes, verifies patient information utilizing multiple hospital computer applications to perform Overlays and review demographic updates ensuring that the changes made were appropriate, to maintain the accuracy and integrity of the patient record. 
  • Oversees and assigns Chart Correction tasks in Epic and OnBase and communicates with other department personnel (physicians, nurses, technicians, etc.) as needed to ensure completion in other ancillary systems in a timely manner.
  • Continuously monitors all Health Information Identity work queues
  • Assists patients with MyChart issues, providing verification codes, password assistance, and proxy access. 
  • Advises supervisor and/or manager of situations requiring followup.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Education, Knowledge, Skills and Abilities Required:

  • High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
  • Minimum of 3 years experience in healthcare or related fields.
  • Excellent organizational and communication skills.
  • Proficient computer skills.
  • Proficient in learning computer systems.
  • Excellent written and verbal communication skills.
  • Proficient computer skills such as Microsoft Office and Google Suite platforms.

Education, Knowledge, Skills and Abilities Preferred:

  • College coursework in Health Information Management or related fields.
  • Minimum of 3 years in a Health Information environment.
  • Knowledge of medical terminology.
  • Chart correction experience.
