Salary: $18 - $25 Hourly

EPIC Health Partners is seeking a compassionate and motivated professionals to join our Mental Health Skill Building team. This position plays a vital role in promoting recovery, independence, and wellnessall under the supervision of a licensed mental health professional. Whether you're an experienced professional or someone just beginning your career in mental health, this is the opportunity for you! Essential Duties & Responsibilities:

  • Deliver person-centered care in homes and community settings.
  • Assist in the creation and ongoing revision of service plans tailored to the needs of each individual.
  • Monitor and ensure that service plans are being effectively carried out.
  • Provide linkage to resources and case management support for individuals.
  • Submit necessary documentation according to established timeframes.
  • Maintain continuous communication with individuals and key stakeholders to ensure coordinated planning and service delivery.
  • Participate in required training and team meetings.


Qualifications:

  • Registered as a QMHP, QMHP-T with the VA Board of Counseling, or certified as a QPPMH
  • Bachelors degree or higher in a Human Services-related field (e.g., Psychology, Social Work, Counseling).
  • At least 1 year of behavioral health experience.
  • Valid drivers license and reliable transportation.
  • Current CPR/First Aid certification and TB screening.


Work Environment:

  • Field-based role requiring travel to client homes and community locations.
  • Must be able to sit, stand, and engage with clients for extended periods.
  • Flexible scheduling, including evenings or weekends, may be required.
