Position Summary:

NeighborHealth is seeking a Population Health Coordinator to join our team, where you will have the opportunity to make a profound impact on the lives of underserved individuals and families. You will provide a critical role by successfully supporting the quality improvement, outreach, clinical operations, and population health efforts of key strategic initiatives within our department and across the health center.
This role is ideal for a professional with knowledge of clinic operations and patient facing care who is looking to grow in quality improvement, population health, and using data to inform decisions to improve patient outcomes and/or workflows.
  • $The role requires a combination of patient outreach and quality improvement work. You will work with patients to identify barriers to care, solve these barriers, and communicate with care teams across the organization. In addition, you will learn evidence-based tools in quality improvement, process improvement, and data usage to improve patient outcomes.

    The successful candidate will be able to perform the following responsibilities:

  • Lead or support clinical operations workflow development and iterative improvement as assigned.

  • Demonstrate the ability, willingness, and motivation to learn and apply population health and quality improvement principles.

  • Support Quality Improvement/Population Health Managers with meeting preparation, data collection and analysis, clinical operations activities, quality improvement initiatives, and workflow development.

  • Build and maintain relationships with key stakeholders across the enterprise as needed or directed.

  • Collaborate with key stakeholders to drive improvements in patient care and patient outcomes as directed.

  • Assist with implementing and monitoring workflow and/or screening fidelity as directed.

  • Oversee volunteers for outreach activities as needed or assigned.

  • Utilize internal and external reports (e.g., Reporting Workbench) to assess patient outreach needs and determine actions to close care gaps as needed or directed.

  • Perform and document patient outreach efforts following NeighborHealth protocols, with the goal of connecting patients to care and improving health outcomes

  • Maintain knowledge of MyChart and patient access workflows.

  • Recognize "red flag" or urgent symptoms and determine when immediate nurse involvement is required.

  • Provide EMR and technology training within the practice; serve as a resident expert on assigned applications/tools.

  • Handle non-call-related functions such as email correspondence as needed.

  • Demonstrate proficiency in EPIC, Word, Excel, Visio, and PowerPoint.

  • Maintain excellent punctuality and attendance.

  • Promote teamwork through self-direction and self-motivation; independently solve problems or seek consultation when appropriate.

  • Display outstanding customer service skills.

  • Adhere to all organizational and departmental policies and procedures.

  • Participate in required departmental and organizational meetings and activities.

  • Navigate EPIC to access relevant information and perform duties including:

  • Handling calls appropriately

  • Employing effective problem-solving skills

  • Scheduling, rescheduling, and canceling appointments

  • Documenting clear, concise messages and routing them appropriately

  • Managing refill requests and encounters accurately and effectively according to guidelines

What You'll Bring

  • High school diploma or equivalent required , bachelor's degree preferred
  • Relevant administrative or support certification preferred, such as Medical Assistant Two years of progressively responsible work experience
  • Ability to learn specialized vocabulary related to medical field
  • Exceptional written and verbal communication skills
  • Ability to positively influence others with respect and compassion
  • Fluency in Spanish preferred
  • Strong work ethic with a proactive, teamoriented approach
  • Comfortable navigating ambiguity using structured problemsolving techniques

  • Willingness to work occasional evenings or weekends to support program needs
  • Proficiency with Microsoft Office (Word, Excel, PowerPoint, etc.)

Salary: Starting at $54,080, and up to $65,000 based on years of relevant experience.
