Job description

Overview
Registered Nurse – Case Manager position in Pocatello, Idaho, United States. Travel assignment starting 06/15/2026 for Weeks, with a weekly pay range of $1,803 to $1,868 per week. No guaranteed hours. Shift is 5x8-hour days. Work setting is a hospital-based case management unit serving adults, pediatrics, and geriatrics.

Responsibilities
- Perform medical necessity determinations and utilization review for admissions, ongoing stays, and discharge planning.
- Lead discharge planning and care coordination, communicating plan-of-care changes to the interdisciplinary team.
- Maintain knowledge of community resources and referrals; anticipate needs using length-of-stay data.
- Document initial screenings and discharge plans, and keep ongoing plan-of-care updates in Epic in a timely, accurate manner.
- Conduct continued stay reviews, appeals and denials as appropriate, ensuring compliance with DRG, CMS guidelines and HIPAA.
- Provide care coordination for adult, pediatric, and geriatric patient populations; monitor a unit with an approximate daily patient load of 20.
Requirements
- Current RN license in Idaho or a valid Compact license in-hand at submittal.
- BLS certification.
- 2 years of case management experience preferred; previous charge experience preferred.
- Epic charting experience required.
- Orientation: 24 hours; onboarding core items and required documentation to be completed within 48 hours of contract signing.
- Ability to work in an on-site hospital setting and collaborate with a multidisciplinary team.
