Our Hospice Nurses care for patients at the end of their lives. Their main responsibility is to help people live as comfortably and independently as possible and with the least amount of pain, in their final days. One of the largest parts of being a Hospice Nurse involves helping patients, and their families, feel more comfortable about death and providing them with the emotional support they need. Our RNs go beyond that and care for the patient's mind, body and spirit while supporting their families and caregivers as well.

  • Make initial nursing evaluation visit to communicate with physician and determine a plan of care.
  • Practice nursing according to Franciscan Alliance and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
  • Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
  • Collaborate with patients and their families, as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience.
  • Assess, collect, and analyze comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care.
  • Develop an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously review and revise the plan of care as patient condition warrants.
  • Communicate timely changes in patient's assessment/ongoing reassessment to providers that warrant consideration and/or a change in the plan of care and maintain and coordinate communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery.
  • Supervise clinical care provided by LPNs and Home Health Aides.
  • Serve in an on-call rotation and provides patient care outside of regular business hours.
Qualifications
  • Associate's Degree- Nursing/Patient Care- Required
  • Bachelor's Degree- Nursing/Patient Care- Preferred
  • 1 Year Home Health or Hospice Care Experience- Preferred
  • Registered Nurse (RN)- State Licensing Board - Required
  • Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required
  • Driver's License- Required
  • Auto Insurance- Required
