Nascentia Health Options seeks a compassionate and detail-oriented Registered Nurse to join our team as an Assessment RN. In this role, you will assess prospective enrollees and existing members to determine plan eligibility and develop person-centered care plans that support long-term health and independence. You will be a key liaison between members, families, and care teams, ensuring services align with regulatory requirements and individual care needs. This is a full-time role that supports business schedule of Monday through Friday from 8am- 4:30pm.

Performance Responsibilities and Standards:

  • Conduct in-home and virtual assessments of prospective and current health plan members.
  • Evaluate clinical needs, functional and behavioral health status, and social determinants of health using UAS-NY and other assessment tools.
  • Educate members, families, and caregivers on plan benefits, rights, responsibilities, and services.
  • Develop person-centered service plans that identify unmet needs and recommend appropriate supports and services.
  • Use motivational interviewing and active listening to understand member goals and build trust.
  • Review member materials, including handbooks, provider directories, and covered services.
  • Maintain detailed documentation in the electronic medical record (EMR) and follow internal quality protocols.
  • Communicate and coordinate with care managers, providers, and other interdisciplinary team members.
  • Ensure all assessment activities comply with DOH and Medicaid Managed Long-Term Care (MLTC) regulations.
  • Attend staff meetings, trainings, and participate in departmental quality improvement initiatives.
  • Serve as a professional representative of Nascentia Health Options in the community.
  • Meet or exceed productivity and scheduling standards.

Qualifications:

  • License: Registered Nurse (RN) licensed and in good standing in New York State
  • Education: Associate degree in Nursing required; BSN preferred
  • Experience:
    • Minimum 1 year of nursing experience preferred
    • Experience with UAS-NY assessments strongly preferred
    • Knowledge of long-term care, home care, Medicaid/Medicare preferred
  • Skills:
    • Strong verbal and written communication skills
    • Proficiency in Microsoft Office and ability to learn EMR systems
    • Independent, detail-oriented, and able to manage a flexible schedule
    • Bilingual abilities a plus
  • Other:
    • Valid NYS driver’s license and reliable transportation
    • Ability to drive and work in various environmental and weather conditions

Work Environment & Physical Requirements:

  • Hybrid role: Primarily home-based with required in-home member visits and occasional in-office meetings
  • Must be able to sit, walk, stand, drive, and perform data entry throughout the day
  • Ability to lift up to 20 lbs frequently and up to 50 lbs occasionally
  • Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.

This is a per-visit position with an estimated annualized range of $80,000–$100,000+, based on visit volume*.

*Actual compensation will vary depending on the number of completed visits per pay period. A minimum average of 20 visits per pay period is required to maintain full-time status.
