Job Description
The Hospice teams at Samaritan Health Services support patients and family members through the final stages of their illness. Evergreen Hospice serves patients in various counties, delivering care primarily in home settings, including residential homes, skilled nursing facilities, assisted living facilities, and adult foster homes. Our services include intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, and bereavement support, with medications and equipment provided as appropriate.
Responsibilities
  • Provide comprehensive nursing care to hospice patients and support their families.
  • Conduct assessments, develop care plans, and manage pain and symptom relief.
  • Collaborate with multidisciplinary teams to ensure optimal patient care.
  • Offer emotional, spiritual, and bereavement support to patients and families.
  • Maintain accurate records and ensure compliance with healthcare standards.
