Job description

Join to apply for the Grant Position Verde Valley Parent Partnership Worker 2 role at Northern Arizona Healthcare.

This position, under the supervision of the Manager/Supervisor, involves providing home-based parenting education, developmental assessments, emotional support, and preserving the medical home for at-risk families.

ResponsibilitiesAdministrative
  • Provide referrals and resources based on family needs.
  • Assist with monthly billing documentation and data collection.
  • Perform clerical duties, including data entry for the home visiting program.
  • Follow up on referrals and issues identified.
Community Outreach
  • Build and maintain relationships within the community.
  • Assist in developing community support and education groups.
Operations
  • Focus on strengthening parent-child relationships and reducing family stress.
  • Coordinate play groups.
  • Perform initial assessments for potential families.
  • Manage referrals and screenings for program admittance.
  • Use tools like the Family Stress Checklist for needs assessment.
  • Collaborate with the Program Manager/Supervisor on referrals.
  • Assist with client satisfaction calls and home observations.
  • Create a safe, trusting environment for families.
Staff Development
  • Support staff training and orientation.
  • Coordinate educational opportunities for staff.
Compliance/Safety
  • Report safety incidents promptly and attend safety training.
  • Ensure compliance with regulations and maintain valid certifications and licenses.
Qualifications

Education: Associate's Degree in Nursing, Social Work, or related health field required; Bachelor's preferred.

Certifications & Licenses: BLS (American Heart Association), valid Arizona driver’s license, Fingerprint Clearance Card (application required upon hire, card within 90 days).

Experience: Minimum 3 years in child development, computer proficiency, home visiting experience preferred, bilingual a plus.

Healthcare environment familiarity and computer skills are essential.
