The Mohawk Trail Counseling Center is seeking dedicated and compassionate Licensed Professional Counselors (LPCs) to join our team in a full-time or part-time capacity. As part of our remote setup, you will provide essential mental health support and guidance to patients in the Beaverton, Oregon area. Key ResponsibilitiesAs a Licensed Professional Counselor at Mohawk Trail Counseling Center, your duties will include:

- Conducting comprehensive patient intakes, focusing on diagnostic and functional evaluations, and collaborating with primary care providers to formulate personalized treatment plans.

- Detecting patients at risk and developing preventive care plans to avoid deterioration of their conditions.

- Documenting patient progress thoroughly in treatment charts and ensuring all information is communicated effectively to primary care providers.
- Implementing behavioral changes and promoting healthier lifestyle choices in patients, particularly those facing physical disorders.
