The Mohawk Trail Counseling Center is seeking dedicated and compassionate Licensed Professional Counselors (LPCs) to join our team in a full-time or part-time capacity. As part of our remote setup, you will provide essential mental health support and guidance to patients in the Beaverton, Oregon area. As a Licensed Professional Counselor at Mohawk Trail Counseling Center, your duties will include:

• Assisting primary care providers in identifying and addressing mental health and psychosocial issues, contributing effectively to the primary care team.
• Conducting comprehensive patient intakes, focusing on diagnostic and functional evaluations, and collaborating with primary care providers to formulate personalized treatment plans.
• Delivering brief, targeted interventions for patients requiring mental health services, ensuring they receive the necessary support in a timely manner.
• Advising primary care providers on the suitability of patient needs to be addressed in a primary care environment versus requiring specialty mental health referrals.
• Initiating regular follow-ups with patients to assess ongoing needs and making adjustments to treatment approaches as necessary.
• Creating relapse prevention plans and assisting patients in maintaining stability and healthy functioning.
• Detecting patients at risk and developing preventive care plans to avoid deterioration of their conditions.
• Coordinating behavioral health services, including connecting patients with external treatment providers with appropriate consent.
• Documenting patient progress thoroughly in treatment charts and ensuring all information is communicated effectively to primary care providers.
• Implementing behavioral changes and promoting healthier lifestyle choices in patients, particularly those facing physical disorders.
• Equipping patients with self-management skills and educational resources that empower them as active participants in their care.
• Consulting with clinic management and team members about behavioral health services and contributing to outcome evaluations and program improvements.
• Supporting patients in adhering to any medical treatments initiated by primary care providers, including coping strategies for medication side effects.
• Abiding by the Code of Ethical Conduct for Clinical Professionals and undertaking other duties as assigned.

• A Master’s Degree in Clinical Counseling or a related field.

• Strong understanding of the social and personal implications of mental health issues for individuals and their families.

• Excellent communication skills, with the capacity to work cooperatively within a multi-disciplinary team framework.
• Sound judgment and the ability to make independent decisions based on professional standards and knowledge.

While not mandatory, the following attributes are advantageous:

• Bilingual proficiency in English and Spanish.
• Experience within community health or Federally Qualified Health Centers (FQHC).

This position offers a competitive salary ranging from USD 87,000 to 105,000 annually, commensurate with experience. Additionally, we provide a sign-on bonus for qualified candidates.
