As a Licensed Clinical Social Worker at our Hospice Center, you will work remotely, allowing for flexibility while still being part of a dynamic multidisciplinary team. • Evaluate and diagnose patients, identifying psychosocial problems that impact their mental and emotional well-being, and create individualized treatment plans.
• Adhere to professional standards as outlined by the National Association of Social Workers (NASW) Code of Ethics, ensuring all practices reflect respect and integrity.
• Conduct thorough bio-psychosocial assessments to determine barriers to discharge and develop strategies to enhance health outcomes and resource utilization.
• Collaborate with medical and support staff to provide comprehensive care recommendations, designed to improve patient treatment adherence and care coordination.
• Provide both individual and group therapy sessions using evidence-based treatment modalities and monitor patient progress critically.
• Document all social work activities and interventions in patients' medical records accurately and consistently, meeting departmental standards.
• Educate patients and their families on medical and social work processes to facilitate understanding and acceptance of treatment plans for optimal functioning.
• Assess advocacy needs and coordinate with community resources to empower clients in achieving self-determination.
• Complete case-related statistics and reports as required to provide insight into patient care and departmental effectiveness.
• Engage in ongoing professional development activities and maintain a current understanding of community resources relevant to mental health care.
• Comply with all departmental policies and regulatory standards, ensuring a safe care environment.

You will provide support to patients facing terminal illnesses, assisting them and their families in understanding the emotional implications of their experiences. • Master’s Degree in Social Work from an accredited institution.
• A current license from the State Board of Social Work Licensure at the Licensed Clinical Social Worker (LCSW) level.
• Demonstrated proficiency in addressing the care needs of diverse populations considering various factors such as aging processes, human development stages, and cultural dynamics.
• A minimum of one year of clinical experience is required, ideally in a health or mental health setting relevant to the populations we serve.
