In this full-time role, you will engage with patients in a supportive and impactful manner, helping them navigate their psychological and social needs during their healthcare journeys. Your expertise will be invaluable as you collaborate with an interdisciplinary team to promote holistic patient care and enhance overall outcomes.

Located in Weymouth, Massachusetts, we operate within a community health clinic framework that focuses on integrated care for a diverse patient population. Our team is composed of skilled professionals who adhere to evidence-based practices and state-of-the-art standards, ensuring the highest quality of care. We pride ourselves on fostering an environment of compassion, innovation, and continuous improvement.

The position is remote, allowing you to provide vital care to patients from the comfort of your own home. This flexibility is paired with a supportive virtual team atmosphere, where collaboration and communication are strongly encouraged. Key Responsibilities

As a Licensed Independent Clinical Social Worker, your primary responsibilities will include:

  • Conducting comprehensive psychosocial assessments to evaluate patients' emotional, social, and financial needs.
  • Working closely with other medical professionals to create individualized care plans that cater to the unique circumstances of each patient.
  • Providing individual and group therapy sessions addressing issues such as adjustment to illness, coping mechanisms, and grief.
  • Coordinating with community organizations to arrange comprehensive post-discharge services and ensure patients receive the necessary support after leaving the clinic.
  • Offering emotional support and educational resources to patients and their families, facilitating informed decision-making about treatment options.
  • Participating actively in hospital committees and meetings to address and provide insights on social work-related initiatives.
  • Providing crisis intervention and immediate support to patients and families in distress.
  • Documenting all patient interactions accurately in compliance with organizational policies and pertinent legal requirements.
Patient Population / Client Focus

You will work with diverse patient demographics, including individuals experiencing mental health disorders, chronic illnesses, and psychosocial challenges. Your role will focus on understanding each patient's unique context, forging strong therapeutic relationships, and building trust to foster a healing environment. You will advocate for patients within their healthcare journeys, helping them navigate their experiences and access essential resources.

Qualifications

The ideal candidate will possess:

  • A Master’s degree in Social Work from an accredited program.
  • Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).
  • Strong communication and interpersonal skills essential for collaboration with healthcare teams and effective patient engagement.
Preferred Experience

While not mandatory, candidates with two or more years of experience in a healthcare or hospital setting will be given preference. Familiarity with medical terminology, mental health disorders, and crisis intervention techniques will be advantageous. Experience utilizing electronic health record (EHR) systems and telehealth platforms is also preferred, as these tools are integral to our clinic's operational workflows.

Compensation and Benefits

We offer a competitive annual salary range of USD 70,000 to 100,000, commensurate with experience and qualifications. Our comprehensive benefits package includes:

  • Health insurance covering medical, dental, and vision needs.
  • Retirement plans with employer matching to bolster your long-term savings.
  • Generous paid time off and holiday allowances to help you maintain a healthy work-life balance.
