This professional will be responsible for delivering primary care in an outpatient clinic setting based on developed clinical protocols. Completion of patient records, billing sheets, and leave requests in accordance with the policies of CHC.

Attend provider meetings and bimonthly management meetings, and participate as requested in Quality Assurance activities. Duties also include on-call telephone coverage after clinic hours in addition to occasional weekends. Requirements include: graduation from an accredited program, nationally certified within one year of completion of program, maintaining national recertification every six years and licensed in the state of Utah in accordance with the Physician Assistant Practice Act.
