Health Care Professional - Patient Navigator

The patient navigator's primary role will be to assist appointment scheduling and access and to coordinate diabetes care for families who are at high risk of diabetes complications.

Key responsibilities include:

  • Identify families in need of patient navigator outreach
  • Outreach to the families via phone, email, portal messages, and in person in the clinic
  • Schedule patient visits
  • Set up Medicaid ride services
  • Refer diabetes management questions to the diabetes educator

Work location: Onsite – this role is expected to work onsite and is located in Aurora, Colorado.

The Barbara Davis Center for Diabetes (BDC) specializes in type 1 diabetes research and care for children and adults. Clinicians, clinical researchers, and basic biomedical scientists collaborate at the BDC to find the most effective treatment, prevention, and cure for type 1 diabetes. Founded by Marvin and Barbara Davis in 1978, it is currently one of the largest diabetes institutes in the world. The Center provides state of the art care to over 7,500 children and adults with diabetes as well as provides inpatient care to patients seen at Children's Hospital Colorado.

Qualifications:

Minimum qualifications:

  • Entry Level: Bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
  • Intermediate Level: Bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution. 1 year of professional experience working in quality improvement within a medical practice.
  • Senior Level: Bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution. 2 years of professional experience working in quality improvement within a medical practice.
  • Substitution: A combination of education and related technical/military/paraprofessional experience may be substituted for a bachelor's degree on a year for year basis. An advanced degree (Masters or Doctorate) may be substituted for experience on a year for year basis if the degree is in a field of study directly related to the work assignment.

Applicants must meet minimum qualifications at the time of hire.

Preferred qualifications:

  • Master of Public Health (MPH) or related advanced degree preferred
  • Experience in quality improvement in a type 1 diabetes clinic or clinical research environment
  • Oral Spanish language proficiency
  • Experience building and running reports in an electronic medical record, preferably the Epic system
  • Advanced knowledge of the Microsoft Office package including Word, Excel, PowerPoint, Access, and Outlook
  • Basic understanding of statistical and data analysis
