Travel Licensed Practical Nurse (LPN)

Warm Springs Service Unit โ€” Indian Health Service | 12-Month Assignment

Location & Assignment

Warm Springs Health and Wellness Center, 1270 Kot-Num Road, Warm Springs, OR 97761

12-month travel assignment supporting the Medical Program at the Warm Springs Service Unit (Indian Health Service). Housing in the local area is limited โ€” this is a designated travel/per-diem eligible assignment.

Schedule: Monday, Tuesday, Thursday, Friday 8:00 AM โ€“ 5:00 PM; Wednesday 12:00 PM โ€“ 5:00 PM (one-hour unpaid lunch on full days). Clinic closed on federal holidays.

What You'll Do
  • Perform patient health and risk assessments within scope of practice
  • Record vital signs and complete health screenings
  • Perform nursing interventions and monitor patient health status
  • Administer medications and treatments within scope of practice and applicable orders
  • Provide patient education and preventive health counseling
  • Coordinate follow-up and preventive care scheduling
  • Assist with post-procedure care and follow-up as authorized
  • Document all patient encounters and nursing services in RPMS/EHR
Qualifications
  • Graduation from an accredited practical nursing program
  • Current, unrestricted LPN license (any State, DC, Puerto Rico, or U.S. Territory)
  • Minimum one (1) year of clinical nursing experience preferred
  • Current BLS certification
Required Documentation (prior to start)
  • Current, unrestricted license/certification in good standing
  • Current BLS certification
  • Immunization records
  • Background investigation (e-QIP) and OF-306 federal employment forms
  • HIPAA training/agreement
  • Successful completion of credentialing and privileging prior to start
