Licensed Practical Nurse (LPN)Location: 119-Southern Utah Veterans Home - Ivins, St. George, UT, 84738Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to residents at the Southern Utah Veterans Home. The LPN works under the supervision of a Registered Nurse (RN) and collaborates with the healthcare team to ensure high-quality, compassionate care for our veteran residents.

Key Responsibilities
  • Administer medications and treatments as prescribed by physicians and according to facility policies.
  • Monitor and document residents’ health status, including vital signs, symptoms, and changes in condition.
  • Assist with the development and implementation of individualized care plans.
  • Provide wound care, catheter care, and other nursing procedures as required.
  • Collaborate with RNs, physicians, and other healthcare professionals to coordinate resident care.
  • Maintain accurate and timely documentation in residents’ medical records.
  • Respond promptly to resident needs and emergencies.
  • Educate residents and their families on health maintenance and disease prevention.
  • Adhere to all facility policies, procedures, and regulatory requirements.
Qualifications
  • Current and valid Licensed Practical Nurse (LPN) license in the state of Utah.
  • Minimum of 1 year nursing experience preferred, preferably in long-term care or geriatric settings.
  • Basic Life Support (BLS) certification required.
  • Strong communication and interpersonal skills.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Compassionate and patient-centered approach to care.
