DETAILED JOB DESCRIPTION
Monitors and responds to patient triage both in person and/or on the nurse phone line.
Assesses life-threatening or crisis situations at the onset of a call and manages referrals to EMS. Based on further assessment and approved guidelines, refers to Emergency Center, Urgent Care, providers office or self-care at home. Follows appropriate evidence-based triage protocol as available and appropriate.
Asks open-ended questions to identify actual or potential health problems. Speaks directly to individual with symptoms whenever possible.
Documents every caller encounter in the electronic medical record (EMR) to include assessed needs, recommended actions and follow-up plan to ensure a high quality of care, effective continuity of care, and prudent risk management for the organization.
Maintains and enhances an up-to-date level of knowledge regarding professional nursing practices and telehealth nursing.
Provide quality care to patients and work with multi-disciplinary team to ensure the highest patient experience.
May perform direct patient care duties within the clinic based on validated competency on the Clinics Primary Care RN Competency Summary Checklist. This may include but is not limited to nurse only visits, in person patient triage, managing patient portal messages and medication refill requests, complex patient education, patient intake, assisting with surgical/diagnostic tests and procedures, collecting specimens/point of care testing and complex dressing changes/wound care.
REQUIRED QUALIFICATIONS
Missouri Board of Nursing RN or Nurse Licensure Compact multi-state RN. When primary state of residency changes, compact state RNs must apply under new state of residency within thirty (30) days.
One year of RN experience.
PREFERRED QUALIFICATIONS
One (1) year of experience working with patients in ambulatory care.
Three (3) years of RN experience.
Additional license/certification requirements as determined by the hiring department
PHYSICAL DEMANDS
The physical demands described here are representative of those that must be met with or without reasonable accommodation. The performance of these physical demands is an essential function of the job. When working in clinic, this position may be very active and require ambulating, reaching, and moving objects weighing up to and over 20 lbs. This role also routinely may come into contact with patients who may have contagious illnesses. This position is in a health care facility open 24 hours a day, 365 days a year. Work during inclement weather will be required.
