Intake Nurse/Social Worker

Cedar Creek Hospital of Michigan is a beautiful and state of the art 72-bed facility. Cedar Creek provides evidence-based treatment to adolescents and adults struggling with mental health issues. Cedar Creek Hospital invests in our patients by offering an integrated medicine approach that provides the latest therapies including yoga, mindfulness, aromatherapy, music therapy and nutrition. As part of our success and innovation in the field we are seeking qualified applicants for an Intake Nurse/Social Worker position.

The Intake staff responds to requests for services and determines clinical appropriateness of each request. Information is normally obtained through initial clinical phone information, followed by clinical evaluation if appropriate for the client and coordinates with the physician regarding patient care needs. The Intake staff will facilitate this process by assisting the client, family and referral sources. They also provide information to clients, families and referral sources regarding treatment programs, insurance and admission procedures. Hours PRN/Per Diem is required to work at least 1 shift per month to maintain employment.
