Essential Duties: Facilitate linkages/coordinate services with outside agencies for patients with unmet needs (i.e., childcare, transportation service, psychiatry, housing, etc.) Track attendance of newly enrolled patients to ensure they complete orientation within allotted days of admission Review FGC program policies in group or individual sessions Compile data to present to medical team, in conjunction with clinical staff input, when doctor intervention is indicated Attend to patients in crisis at the request of medical reception or other staff Work Schedule: Monday - Friday (8:00am - 4:30pm) Start Date: ASAP Work Location: 333 W.
