Job description

Physical Therapist

As a Physical Therapist, you will be responsible for providing and coordinating comprehensive care to patients of all ages through the proper utilization of evaluation, planning and implementation of a treatment plan in accordance with established standards, policies, procedures, protocols and guidelines.

Bachelor's degree in Physical Therapy or entry level equivalent is required. Must be Licensed or license pending in the State of Illinois and without previous disciplinary action in Illinois or any other state. Current CPR card.

Qualifications

Education:

  • Graduate of an approved school of Physical Therapy.
  • Basic knowledge in physical therapy practices in accordance with the P.T. Practice Act.

Licensure/Certification/Registry:

  • Illinois eligible or licensed Physical Therapist.
  • CPR certification is required.

Other Knowledge/Skills/Abilities:

  • Universal Precautions Category I.
  • Must possess valid Illinois driver's license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must show proof of automobile insurance.
Responsibilities
  • Provides rehabilitative care to patients of all ages.
  • Evaluates treatment needs of assigned physician referred patients of all ages
  • Implements the patient treatment according to the program plan, assesses the patient's response and revised the program as needed.
  • Effectively communicates with the patient, family members, hospital staff and the public.
  • Attends patient care conferences and outpatient clinics or sees that the needed information is provided to appropriate personnel for reporting.
  • Keeps timely and accurate medical, administrative and education records.
  • Coordinates effective patient care with the assistance of the Physical Therapy Assistant and Physical Therapy Aide.
  • Participates in training programs for students, hospital staff and the community.
  • Maintains skills in treatment procedure through utilization of inservice and continuing education opportunities and expertise of other clinical staff personnel.
  • Demonstrates effective use of departmental equipment for patient care
  • Contributes to the maintenance of a satisfactory work environment.
  • Promotes health and safety in the work environment.
  • Actively participates in competency assessment process.
  • Performs other related work as required or requested.
