Job description

LVN home health office administrator Overview: The LVN Case Manager Home health setting provides administrative and logistical support to healthcare teams. The assistant helps coordinate patient care, manage documentation, and ensure that patients receive the appropriate services and resources while receiving care at home. This role is vital in streamlining patient care delivery, maintaining communication between the healthcare team, and providing essential administrative support to the administrator.
  • Licensure: An active, Licensed Vocational Nurse (LVN) license.
  • Experience: 1–3 years of clinical or home health administration experience.
  • Technical Skills: Proficiency with EMR software, medical terminology, and typing/general office tools.
  • Knowledge: Familiarity with HIPAA, Medicare Conditions of Participation, and Electronic Visit Verification 
Clinical documentation and record keeping:
  •  Processing, tracking, and verifying physician orders, lab results, and medication.
  •  Ensuring all patient records and documentation strictly adhere to HIPAA and home health regulatory standards.
  • Assist the Case Manager by coordinating clinal documentation.
  • Help prepare, update, and monitor patient care plans, ensuring services are delivered as prescribed.
  • Maintain accurate, up-to-date patient files and Electronic Health Records in compliance with state and federal regulations.
  • Assist with clinical audits and Quality Assurance to ensure standards are met.
  • Monitor adherence to infection control protocols and safety standards. 
Required Qualifications:

  • Education: High school diploma or equivalent required. A degree in healthcare administration is preferred.
  • Experience: Prior experience in home health, case management, or healthcare administration is preferred. 

Skills:
  • Strong organizational and time-management skills.
  • Good communication skills, both verbal and written.
  • Ability to maintain patient confidentiality and follow HIPAA regulations.
  • Familiarity with medical terminology, healthcare processes.
  • Certification in medical office management is a plus.
