Job description

Pharmacist
Responsible for providing clinical pharmaceutical care to patients and for interacting with physicians/health care professionals regarding Disease Management and medication reconciliation.
Duties and Responsibilities:
  • Adherence to company policies and procedures while maintaining a professional, positive, and caring attitude is required.
  • Adhere to Sierra Medical Group's Employee Handbook, Policies and Procedures.
  • Comply with state and federal drug laws as regulated by the California State Board of Pharmacy, the Drug Enforcement Administration, and the Food and Drug Administration.
  • Proactively support and practice company mission, vision and values.
  • Conduct comprehensive medication reviews for patients. Communicate with primary care providers, specialists, and care coordinators to ensure continuity of care.
  • Educate patients and caregivers on proper medication use, side effects, and adherence strategies.
  • Perform accurate and timely medication reconciliation at all transitions of care (admission, transfer, and discharge) Identify and resolve medication discrepancies, duplications, or potential interactions.
  • Develop and manage plans for chronic diseases such as diabetes, hypertension, heart failure, COPD, CKD etc.
  • Adhere to all Compliance Regulations including, HIPAA, Fraud, Waste, and Abuse, Harassment, Code of Conduct, Model of Care, Cultural and Linguistics, and Injury and Illness.
  • Visit face to face with the patients for medications counseling and for treatment regimens, including but not limited to diabetes and other disease conditions.
  • Actively document clinical interventions and recommendations.
  • Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers.
  • Participate in meetings as required by department management.
  • Follow and adhere to standards of attendance policy, as it is pertinent to be available to assist staff and to deliver optimal efficient care to our patients.
  • Use time keeping system appropriately; and notify supervisor in writing when requesting paid time off.
  • Be courteous, flexible, and adaptable.
  • Promote departmental and organizational goals.
  • Know and follow applicable safety standards.
  • Perform other duties as assigned.

Job Requirements:
  • Doctorate in Pharmacy from an accredited institution.
  • Current and active Pharmacist licensure.
  • Hospital Pharmacy Residency or comparable pharmacy experience preferred.
  • Ability to follow and adhere to FDA Health Regulations, and other Legal Compliances.
  • Excellent written and verbal communication skills.
  • Ability to communicate (written/verbal) effectively with patients, families, staff members, and providers.
  • Ability to handle and prioritize tasks in order to meet all given deadlines and productivity goals.
  • Ability to responsibly handle matters of a confidential nature.
  • Ability to multi-task, highly productive environment.
