Now Hiring: Experienced Home Health LVN


Join Our Growing Home Health Team!


We are seeking a compassionate, dependable, and experiencedLicensed Vocational Nurse (LVN)to provide high-quality home health care to patients throughout our Texas service area.


Qualifications


  • Current Texas LVN license in good standing
  • Minimum 1 year of recent home health experience required
  • Strong clinical assessment and documentation skills
  • Excellent communication and time management skills
  • Ability to work independently while providing compassionate patient care
  • Current CPR certification
  • Reliable transportation, valid driver's license, and current auto insurance required


Responsibilities


  • Perform skilled nursing visits in patients' homes
  • Conduct thorough patient assessments and monitor changes in condition
  • Develop and implement individualized plans of care under the supervision of the RN
  • Accurately complete electronic documentation in a timely manner
  • Educate patients and caregivers on disease management, medications, and home safety
  • Communicate effectively with physicians and the interdisciplinary care team
  • Ensure compliance with agency policies and Medicare guidelines
