Job description

JOB SUMMARY:

The Pharmacy Clinical Specialist is responsible for participating and focusing on pharmacy clinical programs. This includes monitoring patient medication regimens and providing appropriate interventions. The Clinical Specialist shall also assist in mentoring pharmacy staff and students and assist in the evaluation and implementation of clinical/operational programs. 

Duties (included but not limited to): 

  • Interpret orders and transcribes or verify them accurately and efficiently into the pharmacy computer system
  • Accurately compound and dispense pharmaceuticals
  • Comply with facility policy and procedures and regulatory requirements
  • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
  • Provide clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
  • Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection
  • Attend and actively participate in daily multidisciplinary and bedside rounds
  • Complete drug utilization reviews to promote rational drug therapy and implement improvements identified
  • Provide accurate and timely medication information to healthcare staff and patients
  • Lead and develop pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Lead/co-lead, develop, manage and mentor antimicrobial management program (AMP).
  • Document all clinical activities and interventions accurately and completely. Responsible for mentoring and education as well as ongoing analysis of interventions to ensure accurate and appropriate documentation and provide summary reports.
  • Support and participate in the goals of the organization’s quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.
  • Participate in the design, collection, and reporting of adverse drug reactions, drug utilization reviews, medication errors and pharmacy quality initiatives
  • Lead the training and mentorship of pharmacists, pharmacy students, residents, technicians, nurses and physicians. Develop and document staff competency on an ongoing basis and develop improvement plans when deficiencies in competencies are identified 

What Qualifications will you need:

  • B.S. or PharmD from an ACPE-accredited School of Pharmacy, a Doctor of Pharmacy from an ACPE-accredited college of pharmacy is preferred. 
  • Licensure/Certification: Active pharmacist license in good standing with the appropriate State Board of Pharmacy is required. BLS/ACLS or PALS licensure is preferred and highly encouraged (AHA or Red Cross)
  • Minimum of 3 years of clinical hospital pharmacy experience
  • Completion of hospital PGY1 or PGY2/pharmacy general practice residency or fellowship
  • Specialty BCPS certification
  • Other certification (e.g FCCP, FASHP, FASCP, CDE, CPE

Doctors Hospital of Augusta, part of HCA Healthcare, is a 350-bed hospital that provides a wide range of medical services.
