Speech Language Pathologist (SLP)
Location: ASVH-Flagstaff, 2100 N. Gemini Rd., Flagstaff, AZ
Schedule: Monday-Friday, 8:00 AM - 5:00 PM | No weekends | Typically 40 hours/week | No overtime
Position Summary:
We are seeking a compassionate
Speech Language Pathologist (SLP) to evaluate and treat residents/patients with speech, hearing, swallowing, and cognitive disabilities. The SLP will develop and implement treatment plans in compliance with state and federal guidelines and under a physician's direction.
Essential Duties:
- Evaluate residents/patients and develop individualized care plans
- Provide direct therapy based on established treatment plans
- Plan and conduct rehabilitative programs to restore communication and/or swallowing abilities
- Administer, score, and interpret speech and hearing tests
- Instruct residents/patients, families, and caregivers on therapy exercises
- Maintain accurate and timely documentation per company policy and HIPAA guidelines
- Collaborate with rehab therapists and nursing staff in a team environment
- Ensure punctuality and regular attendance
Other Duties:
- Develop short- and long-term treatment goals
- Communicate therapy programs to families/guardians
- Maintain written documentation including evaluations, daily notes, and weekly summaries
- Perform other duties as assigned
Qualifications & Skills:
- Bachelor of Science in Speech Language Pathology required
- Active SLP license in the State of Arizona
- Knowledge of effective speech therapy techniques and geriatric care
- Strong clinical skills and collaborative mindset
