Behavioral Health Case Mgr II - Connecticut

Location: Hybrid 1: This role requires associates to be in-office 1 - 2 days per week, fostering collaboration and connectivity, while providing flexibility to support productivity and work-life balance. This approach combines structured office engagement with the autonomy of virtual work, promoting a dynamic and adaptable workplace.

Work Shift: Monday through Friday, 8:30 a.m.-5:00 p.m. (EST)
The Behavioral Health Case Mgr II is responsible for performing case management telephonically within the scope of licensure for members with behavioral health and substance use disorder needs. Provides telephonic behavioral health resource navigation, care coordination, and referral support to individuals seeking behavioral health and substance use services within the ACCESS for Adults program.
How you will make an impact:
  • Partners with providers, patients, and community organizations to identify appropriate behavioral health and community-based resources that meet each individual's clinical, geographic, insurance, and personal needs.
  • Supports individuals in overcoming barriers to care by coordinating referrals, assisting with appointment navigation, and conducting follow-up outreach to promote successful connection to recommended services.
  • Responds to referrals received through the ACCESS for Adults program, assisting with resource identification and appointment coordination, conducting follow-up outreach to support successful connection to services, and maintaining current knowledge of behavioral health, substance use, and community resources throughout Connecticut
  • Uses appropriate screening criteria knowledge and clinical judgment to assess member needs.
  • Conducts assessments to identify individual needs and develops specific care plan to address objectives and goals as identified during assessment.
  • Supports member access to appropriate quality and cost effective care.
  • Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers.
  • Serves as a resource to other BH Case Mgrs.

Minimum Requirements:
  • Requires MA/MS in social work counseling or a related behavioral health field or a degree in nursing, and minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance use disorder treatment; or any combination of education and experience which would provide an equivalent background.
  • Current, active, unrestricted license such as either a RN, LCSW, LMHC, LICSW, LPC , LMFT or Clinical Psychologist to practice as a health professional in the state of Connecticut is required.
  • Previous experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance use and/or medical disorders.
  • Managed care experience.

Preferred Skills, Capabilities and Experiences:
  • Dual licensure in mental health and substance use treatment preferred.
  • Knowledge of Connecticut behavioral health, substance use, and community-based resources.
  • Experience providing telephonic care coordination, resource navigation, or referral services.
  • Demonstrated ability to identify appropriate referrals based on clinical needs, insurance coverage, geographic location, provider availability, and individual preferences.
  • Excellent communication, motivational interviewing, and customer service skills.
