RN Behavioral Health Case Manager | Home Care
Join Our Team as an RN Psychiatric Case Manager with Atrio Home Care!
Atrio Home Care-a partnership of Holland Home and Resthaven-is seeking a compassionate and skilled RN Behavioral Health Case Manager to support clients throughout the Grand Rapids area.
This role focuses on providing individualized psychiatric nursing care to adults in their homes, promoting stability, safety, and independence with a whole-person approach.
Responsibilities
  • Perform comprehensive psychiatric and health assessments for clients and families in their homes and AL & IL facilities.
  • Provide skilled nursing interventions to support stabilization and maintenance of psychiatric and physical health.
  • Collaborate closely with the client's psychiatrist or physician to implement and revise individualized Plans of Care.
  • Counsel clients and families to support health-related needs and understanding of medications, safety considerations, symptoms to monitor, and community resources.
  • Identify, plan, and coordinate therapeutic, preventive, and rehabilitative nursing actions.
  • Prepare accurate, timely clinical notes and ongoing documentation within the EMR system.
  • Communicate changes in condition, needs, and progress to the interdisciplinary team and referring providers.
  • Arrange needed equipment, services, and referrals, including community mental health resources.
  • Provide supervisory visits to home health aides every 14 days.

Location: Grand Rapids region, supporting clients within approximately a 35-mile radius of downtown.

Qualifications
  • Licensed RN in the State of Michigan
  • Graduate of an accredited nursing program
  • 1-2 years psychiatric or mental health nursing experience required; strong knowledge of psychiatric medications preferred
  • Certification in psychiatric nursing preferred
  • Current CPR certification
  • Valid Michigan driver's license, reliable transportation, and auto insurance for home visits
  • Strong communication, assessment, and problem-solving skills; comfort using healthcare technology/EMR systems
