About the Role

A well-established home health and hospice organization is seeking a compassionate Physical Therapist to join its care team across multiple territories in Pennsylvania and Michigan. This is a full-time, community-based role for PTs who enjoy working independently, building relationships with patients in their homes, and being part of a collaborative clinical team that genuinely cares about outcomes.


Key Responsibilities

  • Deliver skilled physical therapy services to patients in residential home settings
  • Develop and implement individualized care plans in coordination with the broader care team
  • Complete clinical documentation accurately and on time using EMR software
  • Manage your own schedule and caseload within the assigned territory
  • Travel locally to patient homes — approximately 20% of your day is driving

Qualifications

  • Bachelor's or Master's degree in Physical Therapy from an accredited program
  • Active PT license in the state where you'll be working (PA or MI)
  • Minimum 1 year of healthcare experience (new grads may be considered)
  • Valid driver's license and reliable transportation
  • Experience with EMR documentation preferred (HomeCare HomeBase a plus)
  • Ability to self-manage time, schedule, and meet charting deadlines
