• Conduct thorough psychosocial assessments for patients to identify emotional, social, and financial challenges they may face, employing standardized assessment tools and clinical interviews.
• Provide effective individual and group counseling sessions focused on issues such as grief, adjustment disorders, and coping mechanisms to assist patients in navigating their healthcare journeys.
• Collaborate closely with medical teams to develop individualized treatment plans that address both medical and psychosocial components of patient care.
• Coordinate post-discharge services by liaising with community organizations, ensuring that patients have the necessary resources for a smooth transition and continued care.
• Offer emotional support and educational resources to patients and families regarding available treatment options, community services, and self-care strategies.
• Participate in interdisciplinary team meetings to discuss patient progress, share insights, and optimize outcomes while maintaining a focus on patient satisfaction and quality of care.
• Maintain accurate and timely documentation in accordance with organizational policies and legal requirements, utilizing EMR systems for efficient record-keeping.
• Engage in crisis intervention tactics when necessary to provide immediate support to patients and families in distress.

As an LCSW at Great Lakes Consultants Services, you will primarily work with a diverse patient demographic, including individuals dealing with acute and chronic health conditions, children, families facing psychosocial stressors, and patients transitioning out of inpatient services. Your expertise will help bridge gaps in care, ensuring that each patient receives not only the medical attention they require but also the emotional and psychological support essential for their healing process.

• A Master’s Degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Virginia.
• Proficiency in mental health assessments and counseling techniques.
• Knowledge of relevant laws, regulations, and ethical standards in social work practice.
• Excellent communication, interpersonal, and organizational skills to facilitate effective collaboration.

• Two or more years of experience working in healthcare or hospital settings, preferably in a community health environment.

• Familiarity with telehealth systems and digital tools used for patient management and counseling.
