This is a fully remote position that offers flexibility to accommodate various schedules, including day, evening, and possible weekend shifts based on departmental requirements. You will collaborate with a multidisciplinary team, including physicians, nurses, and other mental health professionals to ensure seamless patient care. An on-call component may also be part of your role, reflecting the dynamic nature of crisis intervention and support needs.

• Conduct thorough psychosocial assessments to identify emotional, social, and financial issues affecting patients.
• Develop individualized care plans in collaboration with medical professionals to address psychosocial needs effectively.
• Facilitate individual and group counseling sessions, focusing on issues like coping strategies, grief, and adjustment disorders.
• Establish partnerships with community organizations to coordinate post-discharge support services.
• Provide crisis intervention, offering immediate support and action plans during acute emotional distress.
• Educate patients and their families on available resources, treatment options, and coping mechanisms to better navigate their healthcare journeys.
• Offer emotional support to patients and families, listening to their concerns and advocating for their needs within healthcare settings.
• Maintain accurate and timely documentation in compliance with legal and ethical standards, ensuring best practices in patient care.

In this role, you will primarily work with diverse populations facing a range of mental health issues such as anxiety, depression, and trauma. Our community health clinic serves individuals from various backgrounds, providing a culturally competent approach to psychotherapy. You will play a significant role in creating a therapeutic environment that fosters trust and encourages open communication among patients and families.

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong foundational knowledge of mental health disorders and psychosocial dynamics.
• Excellent communication and interpersonal skills, with an innate ability to empathize with patients' experiences.

• Minimum of two years of experience working within a healthcare or hospital setting, preferably in mental health.
• Familiarity with utility of electronic health record (EHR) systems for documentation and patient tracking.
• Experience in crisis intervention methodologies and evidence-based therapeutic approaches.
