Job description

Embassy Manor in Edison, NJ is hiring a full-time Nurse Practitioner (NP) to join our clinical team. For over 20 years, Embassy Manor has been a trusted name in post-acute rehabilitation, long-term care, and memory care in the Edison community — and our Nurse Practitioners are at the center of how we deliver care. Prior nursing home experience is required for this Nurse Practitioner position.

This is a great fit for a Nurse Practitioner (NP) who wants to practice in a clinically sophisticated skilled nursing setting. Our facility runs physician-led specialty programs in cardiac care and CHF, stroke and neurological recovery, wound care, and pulmonary disease management — meaning our NPs manage genuinely interesting, medically complex patients alongside a devoted interdisciplinary team.

What You'll Do

  • Provide direct patient care, assessments, and treatment for residents and short-term rehab patients

  • Collaborate with attending physicians, nursing staff, and therapy teams on care planning

  • Manage medically complex patients within our specialty clinical programs

  • Support hospital readmission prevention and smooth care transitions

  • Document thoroughly and communicate with families as part of a resident-centered approach

Qualifications

  • Active NP (Nurse Practitioner) / APN (Advanced Practice Nurse) license in New Jersey

  • Minimum 6 months of experience as a Nurse Practitioner (NP)

  • Must have prior experience in a nursing home / SNF (Skilled Nursing Facility) / LTC (Long-Term Care) setting — this is required
