Program Overview:
Project Renewal's Healthcare Department is seeking a team-oriented, customer-focused, and strong multi-tasker for the Outreach Specialistposition! Reporting to the Senior Manager for Healthcare Services and Engagement in the Healthcare Department, the Outreach Specialistin close collaboration with clinical team members, program and facility staff to ensure that critical administrative and operational needs for the clinic are being met.
Essential Duties and Responsibilities:
The essential duties of the Outreach Specialist include but are not limited to the following activities:
  • Expanding awareness of all healthcare services available through shelter-based clinics and mobile health units by conducting community outreach throughout shelter residence common areas and surrounding perimeters
  • Build relationships with shelter-based case managers and other client engagement groups across the healthcare-based programs (e.g. substance use treatment programs, etc.) and support processes to ensure clients are connected to service appointments within these programs
  • Support the registration of new patients into the healthcare program using patient registration and appropriate consent forms and entering data into the Electronic Medical Record (EMR), as well as ensure demographic and contact information for existing records are accurate and up-to-date
  • Support the scheduling of patients for medical appointments for behavioral, substance abuse and primary care services with clinics and /or mobile health vans as required
  • Support access to health insurance by facilitating communication between patient and enrollment specialists
  • Provide printed and phone-call based appointment reminders to patients with upcoming appointments; partner with shelter program staff and resources to ensure appointment reminders for appointments made for shelter-based client community are received
  • Promote access to referral-based specialty care, including but not limited to providing referral appointment reminders to patients, conduct follow-up calls for consult notes
  • Facilitate set up and exit processes for scheduled telehealth and telepsychiatry sessions with patients
  • Escort patients to appointments when required
  • Refer calls for prescription and prescription refill requests through the use of telephone encounters to providers
  • Conducting on-going research and compilation of health literature and related resources, electronically and in-print, for distribution and use by both provider-based and patient-based audiences
  • Creating and disseminate health promotion materials on a wide array of health topics in a variety of formats (eye-catching posters, resource guides, and pamphlets) across clinic and mobile health unit settings of care
  • Distributing and collecting semi-annual post- service patient satisfaction surveys
  • Protects patient confidentiality, making sure protected health information is secured
  • Conduct other related duties as required

Qualifications:
  • High school diploma or graduation equivalency degree (GED)
  • Minimum one (1) year work experience in a medical office setting
  • Basic competency in Microsoft Office (Word, Excel)
  • Substantial telephone etiquette, verbal and written communication skills
  • Strong time management, multi-tasking, organization, scheduling skills
  • Strong customer service / relationship management skills

Preferred Qualifications:
  • Associates Degree
  • Knowledge of clinical procedures usually obtained from a certificate or Associates degree
  • Knowledge of office procedures usually obtained from a certificate or Associates degree
  • Experience with the eClinical Works (eCW) electronic health record system a plus
  • Experience working with individuals facing mental and/or substance use disorders, and/or homelessness
  • Bilingual / fluency in Spanish a plus
