Our Enhanced Care Management (ECM) program connects vulnerable individuals to the medical, behavioral health, housing, and community resources they need to thrive. As a Registered Nurse (RN) Clinical Consultant, you will play a critical leadership role in improving health outcomes, reducing barriers to care, and supporting some of our community's most high-need members.

What You'll Do
  • Provide clinical oversight and guidance to Care Managers, Housing Navigators, and interdisciplinary care teams.

  • Support members with complex medical, behavioral health, and social needs through the Enhanced Care Management (ECM) model.

  • Collaborate with Primary Care Providers, Behavioral Health providers, hospitals, specialists, and Managed Care Plans.

  • Review and approve assessments, care plans, and care plan updates.

  • Assist with care transitions, hospital discharge planning, medication reconciliation, and readmission prevention.

  • Provide clinical consultation for high-risk and complex cases.

  • Participate in case conferences, chart reviews, and quality improvement initiatives.

  • Support outreach efforts, including home visits, hospital visits, shelters, and community-based settings as needed.

  • Monitor documentation, outcomes, and compliance with ECM and Managed Care requirements.

  • Train, mentor, and support staff to ensure high-quality, member-centered care.


Work Location & Schedule

  • This is a full-time, primarily in-office position based in our Pomona, CA office, working Monday through Friday from 8:00 AM to 5:00 PM.

  • To support our mission of improving health outcomes through Enhanced Care Management (ECM), the Clinical Consultant will collaborate closely with care teams, providers, managed care partners, and community organizations. Occasional community-based activities, including member outreach, home visits, hospital visits, and provider engagement, may be required to ensure members receive the comprehensive, coordinated care they need.

Qualifications
  • Active California Registered Nurse (RN) license.

  • 3+ years of healthcare management experience or 4+ years of clinical nursing experience in home health, primary care, managed care, care management, or community health.

  • Prior supervisory, leadership, or administrative experience preferred.

  • Experience in Care Management, Case Management, Population Health, Behavioral Health, Managed Care, ECM, or CalAIM strongly preferred.

  • Excellent communication, clinical decision-making, and care coordination skills.

  • Valid California driver's license and reliable transportation.
