Lead Care Manager (ECM) - Hybrid Role
Heritage Health Network (HHN)
Join Heritage Health Network and make a meaningful impact in your community as a Lead Care Manager supporting members through our Enhanced Care Management (ECM) program.
This is a hybrid position (2 days remote, 3 days in the field), giving you the flexibility to work from home while building strong, in-person connections with members across High Desert regions.
Key Responsibilities
  • Serve as the primary point of contact for members and coordinate their care
  • Develop and manage personalized care plans with members and providers
  • Collaborate with healthcare teams and community resources
  • Support members through education, advocacy, and engagement
  • Identify and address gaps in care
  • Maintain accurate, timely documentation

Requirements
  • Experience in care or case management (community-based preferred)
  • Knowledge of Medi-Cal and ECM strongly preferred
  • Strong communication and interpersonal skills
  • Ability to work in a team-oriented environment
  • Willingness to travel within Orange County
  • Bilingual (English/Spanish) preferred
