This role is fully remote, allowing for a flexible work schedule. • Deliver high-quality social work services via remote platforms, providing therapeutic interventions tailored to each client’s unique situation.

• Conduct thorough assessments to understand the psychosocial needs of clients and develop personalized treatment plans in accordance with their goals and preferences.

• Serve as an advocate for clients, connecting them with vital resources and services that support their health, well-being, and rights.

• Collaborate with multidisciplinary teams, including medical professionals and mental health counselors, to coordinate comprehensive care plans that address both physical and emotional health.

• Regularly monitor client progress and adapt treatment strategies in response to evolving needs, ensuring ongoing support throughout their care journey.

You will primarily work with individuals and families facing end-of-life issues and emotional distress associated with hospice care. This population can include patients with terminal illnesses, their caregivers, and loved ones who require support as they navigate complex emotional landscapes. Your role will be pivotal in helping clients find solace, dignity, and clarity during these challenging times.

• A Master’s degree in Social Work (MSW) from an accredited institution, showcasing a solid academic foundation in social work principles.

• Must possess a valid Licensed Clinical Social Worker (LCSW) certification in Oregon, demonstrating the essential credentials to practice independently.

• A minimum of three (3) years of professional social work experience, preferably in a clinical or hospice environment.

• Exceptional communication skills, with the ability to exhibit empathy and compassion while effectively solving problems under pressure.

• Proficiency in using remote working technologies, facilitating smooth interactions with clients and team members through telehealth platforms.

Experience with hospice or palliative care settings is strongly preferred, as well as familiarity with community resources available to support clients at the end of life. Understanding the emotional and psychological aspects of grief and loss will further enhance your effectiveness in this role.

Breathe Easy Counseling Solutions offers a competitive salary range of $100,000 to $105,000 per year, commensurate with experience and qualifications.
