About the Opportunity

A premiere rehabilitation and skilled nursing facility in Staten Island, NY is seeking an experienced Director of Social Work (DSW) to lead our Social Services Department. Our facility is dedicated to providing a warm, nurturing environment that maintains each resident's dignity, individuality, and independence as part of a trusted healthcare network.

Key Responsibilities

  • Direct, establish, and plan overall department goals, policies, and procedures.

  • Oversee resident assessments, personalized care planning, and transition/adjustment support.

  • Lead, mentor, and supervise Social Services Department staff.

  • Manage discharge planning initiatives starting from resident admission.

  • Perform occasional community outreach to foster local partnerships.

  • Apply extensive clinical judgment, creativity, and industry best practices to achieve quality-of-care goals.

Qualifications & Requirements

  • Education/Licensure: Licensed Social Worker (LSW) or Master of Social Work (MSW) required.

  • Experience: 2+ years of supervisory or management experience in a Long-Term Care (LTC) setting.

  • Knowledge: Deep understanding of applicable State and Federal healthcare guidelines and regulations.

  • Skills: Exceptional interpersonal, communication, and leadership capabilities.
