ofcare and make necessary revisions to address all problems in the plan of care.They provide coordination of the plan of care, maintaining continuity of clientcare with other health professionals while identifying interdisciplinary careneeds and making recommendations for additional home health care services.
Job Qualifications:
• Graduate of an accredited School of Nursing with afour-year degree
• Current nursing licensure by the State of Georgia
• One year of skilled nursing experience preferred
• Willingness to accept supervision and instruction asneeded
• Pass a criminal history clearance prior to working
• Comply with the random drug testing program as requested
• Be committed to attendance as scheduled, and open toconsider fill-in
