GENISCi Health and Wellness (GENISCi) is a Virginia-licensed home care organization providing coordinated in-home skilled care, clinician-led healthcare support, care coordination, whole-health services, and technology-enabled health and wellness solutions.

Our care model brings together licensed clinical oversight, qualified in-home care personnel, client and family engagement, and responsible use of health information to support safety, function, independence, and continuity of care.

We are seeking qualified Certified Nurse Aides and Home Health Aides who value professional accountability, clinical teamwork, accurate documentation, and respectful client-centered care.

Position Summary

The Certified Nurse Aide or qualified Home Health Aide provides in-home support to clients receiving skilled or healthcare-related services under the direction and supervision of a GENISCi-designated Virginia-licensed Registered Nurse.

This role supports the safe and effective delivery of skilled services through assigned clinical observations, functional assistance, client-specific care activities, documentation, and timely communication with the supervising RN.

Employees perform only services that:

  • are permitted by applicable law and GENISCi policy;
  • are included in the client’s applicable written clinical plan of care and client-specific instructions;
  • have been properly assigned by an authorized licensed healthcare professional or delegated by a Virginia-licensed RN when nursing delegation is required; and
  • are within the employee’s verified education, training, and demonstrated competency.

This position does not independently perform nursing assessments, diagnosis, treatment planning, clinical triage, or other activities requiring professional nursing or medical judgment.

Key Responsibilities

Under RN direction and client-specific written instructions, responsibilities may include:

  • Taking and recording blood pressure, pulse, respiration, temperature, weight, and other assigned measurements.
  • Measuring and documenting fluid intake and output.
  • Observing and reporting changes in the client’s condition, functional status, behavior, environment, or response to services.
  • Promptly communicating abnormal findings, safety concerns, incidents, or changes in condition to the supervising RN.
  • Assisting with prescribed exercises, range-of-motion activities, ambulation, positioning, mobility activities, and authorized equipment.
  • Performing properly delegated nursing tasks after documented instruction and competency verification.
  • Providing clinically directed assistance with personal hygiene, dressing, toileting, feeding or hydration, positioning, transfers, ambulation, and mobility when those services support an active skilled-care engagement.
  • Maintaining the immediate care area and client equipment in a clean, orderly, and safe condition as necessary to provide assigned skilled-service support.
  • Providing medication reminders or permitted assistance with normally self-administered medications as specifically authorized.
  • Completing accurate, timely, objective, and legible service documentation on the date services are provided.
  • Using GENISCi’s approved electronic documentation and health-information systems only as trained and authorized.
  • Participating in supervisory visits, competency reviews, case conferences, shift reports, and quality-improvement activities.
  • Promoting client dignity, independence, safety, comfort, and participation in care while carrying out assigned skilled-service support.
  • Maintaining timely communication with the supervising RN and designated care-coordination personnel.

Functional assistance under this position is provided as a supportive component of GENISCi’s skilled-care services and applicable clinical plan.

Safety, Privacy, and Professional Conduct
  • Follow standard precautions, infection-prevention procedures, GENISCi's exposure-control plan, and all PPE requirements.
  • Protect client privacy and confidential health, financial, and personal information.
  • Follow emergency, incident-reporting, abuse and neglect reporting, fall-prevention, and workplace-safety procedures.
  • Maintain professional boundaries and refrain from accepting inappropriate gifts, borrowing money, conducting private transactions, or performing unauthorized services for clients.
  • Treat clients and families with courtesy, dignity, respect, cultural sensitivity, and consideration.
  • Immediately report suspected abuse, neglect, exploitation, unsafe conditions, or other reportable concerns according to GENISCi policy and applicable law.
Scope and Professional Accountability

The aide may not independently:

  • perform a nursing or medical assessment;
  • diagnose or identify a medical condition;
  • develop or revise a plan of care;
  • conduct clinical triage;
  • make independent clinical referrals;
  • recommend or alter medications or treatment;
  • accept verbal medical orders;
  • perform tasks outside the employee’s verified training, competency, written assignment, or lawful scope; or
  • represent themselves as a nurse, physician, or other licensed healthcare practitioner unless they independently hold that credential.

STOP-WORK REQUIREMENT: When uncertain whether a task is authorized, the aide must stop and contact the supervising RN before proceeding.

Minimum Qualifications

Applicants must:

  • Be at least 18 years of age.
  • Be able to speak, read, write, and understand English sufficiently to follow instructions, communicate changes, and complete documentation.
  • Meet an applicable Virginia qualification pathway for a home attendant assisting with skilled services, such as active Virginia CNA certification, qualifying home health aide training or competency evaluation, approved nurse-aide education, or qualifying nursing-program education and clinical experience.
  • Provide documentation of required education, certification, training, and competency.
  • Complete required sworn disclosure, criminal-background, registry, credential, exclusion, and other applicable screening.
  • Meet GENISCi health-screening and infection-control requirements, including TB screening when applicable.
  • Complete GENISCi orientation, competency validation, privacy and security training, infection-control training, emergency-procedure training, and client-specific instruction.
  • Maintain CPR and first-aid certification when required by GENISCi policy or the assigned client’s needs.
  • Have reliable transportation and meet applicable driving and insurance requirements.

The employee’s final position title will reflect the credential and qualification pathway verified by GENISCi.

Preferred Qualifications
  • Current Virginia CNA certification in good standing.
  • At least one year of experience in home health, skilled home care, hospice, rehabilitation, long-term care, assisted living, or another healthcare environment.
  • Experience supporting older adults, individuals with disabilities, or clients managing chronic or complex health conditions.
  • Experience using electronic care documentation or health-information systems.
Working and Physical Requirements

Work is primarily performed in private residences and other authorized client living environments. Schedules may include days, evenings, nights, weekends, holidays, or on-call assignments depending on client needs.

The position may involve occupational exposure to blood, bodily fluids, infectious materials, environmental hazards, pets, smoke, clutter, or challenging household conditions. GENISCi provides applicable training, personal protective equipment (PPE), exposure-control measures, and post-exposure procedures.

Employees must be able to perform the position’s essential functions, with or without reasonable accommodation, including standing, walking, bending, reaching, pushing, pulling, assisting with client mobility and transfers, using approved transfer equipment, and lifting or supporting up to 50 pounds when the assignment can be performed safely.
