Hospice RN Opportunity

Puget Sound Home Health and Hospice is seeking an experienced and caring full-time Hospice Registered Nurse Case Manager (RN) in Southwest King County, Washington!

$115,000 to $125,000 annually, DOE

Essential Duties

  • Perform patient assessments (initial + ongoing)
  • Develop and update care plans
  • Administer meds and manage symptoms
  • Educate/support patients and families
  • Attend/participate in IDT meetings
  • Make home visits as needed
  • Document in EMR accurately and timely
  • Communicate with physicians and team
  • Triage urgent calls/on-call rotation
  • Guide on hospice philosophy/DNR/advance directives
  • Provide end-of-life emotional support
  • Mentor new staff
  • Ensure compliance with regulations
  • Coordinate with outside services (DME, pharmacy, etc.)

Requirements and Qualifications

  • Registered Nurse (RN) licensure
  • Prior experience working in a hospice agency is strongly preferred
  • Ability to work independently and demonstrate critical thinking
  • Current BLS certification from AHA
  • Be a licensed driver with an insured automobile in good working order
