Job description

Citizens Memorial Hospital (CMH) is seeking a Social Worker to care for our patients at the main hospital in Bolivar, MO — someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve. 

Position Details 

  • Status: PRN
  • Work Shift: Varies/Fill-In
  • Hours: 15 hours per pay period 

What Your Day Looks Like: You will provide social services to hospital and emergency room patients, working collaboratively and interdepartmentally as an interdisciplinary care team.

As a Social Worker, you will be:  

  • Developing assessments which examine the current problem(s) as well as relevant biopsychosocial and environmental factors
  • Assessing characteristics of patients' support systems, socio-economic needs, and spiritual/cultural needs to develop action plans for additional support, as applicable 
  • Performing care management functions, including referrals to appropriate healthcare, social agencies, and community-based organizations
  • Providing information and referrals to patients and their families regarding the biopsychosocial aspects of patients' adaptation to diagnosis, treatment, and recovery expectations
  • Providing discharge planning in coordination with the interdisciplinary team to facilitate continuity of care by developing a discharge plan with patient and family, documenting all discharge planning in the chart, and documenting a minimum of once weekly on admitted patients

Qualifications

  • Education: Bachelor's degree in Social Work or Bachelor's in related field such as Psychology, Counseling, Gerontology, Sociology, or Human Services; Master’s Degree preferred

  • Skills: Patient care, customer service, listening

  • Licensure/Certification: CPI Training within one year of hire, renewed annually

  • Physical Requirements: Ability to move between floors and patient rooms, prolonged periods sitting and using a computer, infrequent light lifting required
